Part 1 of 2 - Safety Waiver:
I, the undersigned, assume all risk of personal injury and damage to personal property sustained during any events held by Prismatic Arts & Productions, and release the previously mentioned group from any and all liability for any injury, death, or damages, suffered by attending an event, whether due to negligence of the Released Parties, their respective agents, or peers.
I agree to defend, indemnify, and hold Prismatic Arts & Productions, and any of their staff members, and other Released Parties harmless from and against any and all actions, suits, claims, demands, causes of actions, proceedings, losses, costs, expenses including, without limitation, all attorney fees and disbursements, damages, liability, and fines, or penalties, in any way arising out of, or relating to, connected with directly or indirectly, the use of the Premises regardless of whether there is active or passive negligence or fault on the part of the Prismatic Arts & Productions, their staff members, or the other Released Parties.
Furthermore, I hereby certify that the answers given on this forum are complete, true, and correct in every particular. I realize that if any of the foregoing answers made by me are false, I may be removed from the event without refund or compensation.
I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.

Part 2 of 2 - Media Release Agreement
I, the undersigned, do hereby consent and agree that Prismatic Arts & Productions, and its agents, or staff have the right to take photographs, videotape, or digital recordings of me beginning on January 1st, 2026, and ending on December 31st 2026, and to use these in any and all media, now or hereafter known, and exclusively for the purpose of marketing and entertainment. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.
I acknowledge that at all times Prismatic Arts & Productions will retain the copyright subsisting in the images and are the owner of all negatives or digital files from which the said images are directly or indirectly derived. Furthermore, Prismatic Arts & Productions reserves the right to create alterations or composite representations of the media for any lawful purpose without restriction or further compensation to the undersigned.
Prismatic Arts & Productions with their staff and agents, agree not to use the images, recordings, or video tape of the undersigned in whole or in part to promote or sell sex, or to be used in conjunction with pornography, violence, or illegal/illicit activities.
I do hereby release to Prismatic Arts & Productions, its agents, and staff, all rights to exhibit this work in print and electronic form publicly or privately and to market and sell copies. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used.
I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback.
I also understand that Prismatic Arts & Productions is not responsible for any expense or liability incurred as a result of my participation in this recording, including expenses due to any sickness or injury incurred as a result.
I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
